Allergy and Immunology Training Program

Fellowship Application

Thank you for your interest in our Allergy/Immunology Fellowship Program. We are participating in the ERAS for 2009 and you may go to the ERAS website: (www.aamc.org/audienceeras.htm) to complete an application form.

In addition to the completed ERAS on-line application, we also ask that you complete the following:

  1. USF training program-specific application which can be downloaded below.
  2. Letters of recommendation from:
    1. Dean's letter from the medical school you attended
    2. Program Director or Co-director of your current or past training program
    3. Three other mentors from your current or past training program
  3. Curriculum Vitae
  4. Medical school transcripts

Letters of recommendation can be sent to ERAS as part of the standard application. However, the USF training program-specific application and other supporting documents should be sent or emailed directly to Ms. Amy Kramer, Training Program Secretary.

The closing date for 2009 applications is December 31, 2007.

All Children's Hospital, a 216-bed teaching hospital, consists of more than 150 pediatric specialists and subspecialists who service over 40 medical and surgical specialty divisions providing state-of-the-art care for children. There are 72 pediatric residents in training at this time at All Children's Hospital.

Bayfront Medical Center is a 502-bed hospital, staffed by board-certified faculty physicians in Family Practice, Pediatrics or Internal Medicine, as well as other physicians who are pursuing three years of advanced training in the specialty of Family Practice. The highly rated Family Practice Residency at Bayfront is also affiliated with the USF College of Medicine.

The Allergy/Immunology Training Program in the USF Department of Internal Medicine is located at the James A. Haley Veterans Administration Medical Center in Tampa, Florida . This program, under the direction of Dr. Richard F. Lockey, has offered broad-based, accredited, 2-year fellowship training since 1976.
